  ReStructuredText fixes

- doctests are not literal blocks

- bulleted lists do not need to be indented

Incidentally there are two decoys at the end of the README that pretend
to be doctests but aren't.  Adding the third '>' symbol to make them
into proper doctests shows that these tests fail.
